  • PSA Birth Certificate (copy)

  • Latest Report Card (Form 138)

  • Good Moral Certificate (from previous school)

  • 2 pcs 1×1 ID photos (name at the back)

  • Any additional requirements will be posted on the Official Announcements page. Submit at the Registrar’s Office or via the online instructions in our Admissions section.

How much are the fees and how can we pay?

Fee schedules are posted per school year. We accept cash, bank deposit/transfer, and designated e-payment options. Keep your proof of payment and student’s full name/grade level on the reference. For installment plans or assistance, contact the Cashier’s Office.

Are scholarships or discounts available?

Yes. Roosevelt offers limited merit, need-based, and sibling discounts, as well as activity-linked grants when available. Qualification, slots, and deadlines are announced on the Official Announcements page. Submit applications with complete supporting documents to Guidance/Registrar as instructed.

What is the school schedule and annual calendar?

The official calendar (start/end of classes, holidays, exams, PTCs) is posted on this site each year. Section advisers release class timetables before opening week and update families if changes occur. Any emergency adjustments will appear on the Official Announcements page first.

What is the uniform and dress code policy?

Guidelines are in the Student Handbook. Official uniforms and PE uniforms are required on designated days; shoes, grooming, and ID rules apply. Watch for posted “wash day” or special attire advisories on the Announcements page. For questions or accommodations, coordinate with your adviser.

How do we report absences, late arrivals, or early pick-ups?

Notify your adviser before homeroom (or as soon as possible for emergencies). Submit an excuse letter/medical note on return. Teachers will coordinate make-up work and deadlines. Repeated tardiness/absences follow the Handbook’s interventions and approval process.

What clubs, teams, and activities can students join?

Roosevelt hosts academic clubs, service groups, arts and culture, campus ministry, and athletics. Sign-ups typically open during the Clubs & Orgs Fair each term. Some groups require tryouts or interviews; schedules and requirements are announced by the Office of Student Affairs.

Where do we get official updates and urgent advisories?

This website’s Official Announcements page is the authoritative source, echoed on our verified channels linked in the footer. If a message is not posted there, treat it as unofficial. We will never ask for passwords, OTPs, or direct payments via personal accounts.

How do we request student records (Form 137/138, Certificates, COG, etc.)?

Submit a request to the Registrar (in person or via the online form if available). Include the student’s full name, grade level/year graduated, purpose, and a valid ID. Standard processing times apply; peak periods may require additional days. You’ll be notified once documents are ready for release or forwarding.
